The quest for safer and predictable surgical refractive procedures, most notably for patients with moderate to high refractive errors, is an endless battle. However, the best “new” procedure may be one that is recycled from technology started in the 1950s. Early clinical trials have shown that placing a lens in the anterior or posterior chamber of an aphakic patient has resulted in tremendous stability, predictability, and yet reversibility. Phakic intraocular lenses are poised to become an alternative for our most difficult to treat refractive patients.
