Transcripts/Credit Letters

Choosing the Right Form

There are different forms to choose from when you need a transcript or credit letter. Please read the following choices to determine which form you need.

Currently Enrolled Students

This form is to be used by currently enrolled students who need a copy of their academic transcript, immunization records and/or NBEO scores. There is no charge for this service.

Alumni and Former Students

This form is to be used by alumni and former students of the College who need a copy of their academic transcript. The cost per copy is $20 for this service.

CE Participants

This form is to be used by individuals who need a copy of their Continuing Education transcript and/or a Credit Letter. The cost is $20 per transcript copy or $25 for a Credit Letter if requested more than 90 days following the completion of the course.
